Clinical pathways provide a standardized, evidence-based approach to care for specific conditions or procedures. They translate best practice into clear, coordinated steps — helping patients move more smoothly through their care journey and supporting teams to deliver consistent, high-quality care across the province.

Each pathway connects clinicians, operational leaders, and support services around a shared process of care. By aligning people and practices across settings, pathways reduce unnecessary variation, strengthen communication, and improve both outcomes and patient experiences.

Pathways are not policies or checklists — they are living frameworks that evolve as evidence and practice evolve. Through SHA Innovates, teams receive support to design, test, and sustain pathways that are practical, credible, and scalable across the health system.
