The ICRA Clinical Procedure is in final approval stages. The necessary clinical documents will be linked below when available.
ICRA Health Record Visual Indicator
- Paper document for use in health record/chart during current episode of care.
- Used for all clients who have an assessed risk for violence based on VAT score.
- Placed at the front of the purple sleeve.
- Transitional document that is removed and shredded/confidentially recycled at discharge or when the visual indicator is removed.

ICRA Client Pyramid Plan (CPP)
- Individualized ICRA care plan that identifies triggers, contributing factors and successful de-escalation strategies.
- Encouraged for all clients with an assessed risk.
- Information may include staff knowledge of the client as well as information obtained in collaboration the client/family.
- Placed/kept in the purple sleeve behind the Health Record Visual Indicator.
- Retained in the purple sleeve at discharge as part of the permanent health record.

- It outlines the SHA’s commitment to violence prevention initiatives and explains the purpose of the visual indicators and Client Pyramid Plan.
- Includes workbook pages to help the client/family identify triggers, contributing factors and successful de-escalation strategies which can then help to inform the collaborative development of a Client Pyramid Plan.
