COPD Referral
Respirologists, family doctors, and nurse practitioners can refer any person in the province using the Provincial Respiratory Referral Form. Additionally, people living with COPD have the option to self-refer for support services, education, commercial tobacco cessation, pulmonary rehabilitation, and both clinic and home visits.
Provincial Respiratory Referral Form
