General Patient Resources
The Advance Care Planning Program has two booklets that contain guidance and templates for creating a health care directive (HCD) or appointing a proxy. Patients do not have to use an SHA template to create a HCD.
The ABCs of ACP contains a proxy appointment form and a template for creating a general HCD. It can be partnered with Let's Talk Advance Care Planning for additional education.
Future Health Care Decisions contains a proxy appointment form and a template for creating a specific HCD. This booklet should only be provided to patients with a specific health condition who have a good understanding of their current health and are planning for things that may happen due to that condition. Please read the Practitioner's Guide and have a conversation about the anticipated progression and outcomes of the condition before providing this booklet to patients.
Not every individual will require a health care directive. Many people can have their best interests met by identifying their legal nearest relative or appointing a proxy and having ongoing conversations with that person about their health care wishes, fears, and values.
