Currency and Accuracy of Clinical Documents
The accuracy and currency of your document is the responsibility of the owner.
In order to request revisions for a provincial document - Owner of Document to Request
- Contact Clinical Standards to obtain the published editable version.
- Use red font to identify revisions made to the document.
- Include a brief summary of the updates (on submission form or in email).
- Complete the Revision Submission Form below, then submit to Clinical Standards.
- If revisions are extensive, include Project Overview (SHA 0423).

Not the Owner But Have Revision Suggestions
- Email suggestions for revisions to Clinical Standards.
- Clinical Standards forwards any suggestions to the document owner for consideration.
