What is large scale change?

The SHA Large Scale Change team
- Use knowledge of complex system theory to support sustainable large-scale change.
- Create repeatable processes, structures, spaces and approaches that mobilize ideas to action across the SHA.
- Contribute to emerging system needs whose outcomes enable transformation. We do this by partnering with leaders, working cross-functionally, applying project management best practices and using change and implementation practices.
Our Work
Some of the work the Large Scale Change team has done includes, but is not limited to:
- Develop and host Integrated Team Based Care Showcase in 2023, 2024, 2025.
- Lead co-design of Health Network maturity assessments in 2023, 2024, 2025. Partner to develop support tools and resources.
- Establish and maintain the Health Network SharePoint that includes key resources that advance Health Networks.
- Partner to develop Health Network data and service profiles.
- Partner to develop Patient’s Medical Home Discovery Café that aids collaboration to advance Patient’s Medical Homes in Health Networks.
- Partner with the Saskatchewan Medical Association to develop the Patient’s Medical Home Development tool: a guide for clinics.
- Support SHA teams and clinics advancing Patient’s Medical Homes through a project stewardship approach.
- Apply implementation science to improve visibility and coordination of various province wide projects including development of an Implementation Hub available to leaders on Leader Resources (only accessible to users with Leader Resources access).
- Support the development of a new model of central intake and navigation for Mental Health and Addiction Services (MHAS). This included the creation of a workplan, tracking processes, facilitation flows, activities for MHAS Services leadership and working groups.
- Partner to develop the Truth and Reconciliation Conversation series.
- Open our internal Change Navigators orientation and reflection program to interested partners.
- Developed an audio series to inspire each other with the stories or local teams advancing Health Networks.
What to expect when partnering with S&I Large Scale Change team
The LSC team co-creates repeatable processes, structures spaces and approaches that mobilize ideas to action. We partner to address emerging system needs whose outcomes enable system transformation. Our work is often with Primary Health Care teams to advance Health Network development. In addition, the team works with Mental Health and Addictions teams, Acute Care teams and others across the SHA.
We bring a broad skill set to projects including project management, facilitation, tool development, resource curation, report writing, and alignment to SHA goals and priorities.

Health Networks
The S&I Large Scale team has been an integral partner in Health Network development.
The team has:
- Operationalized network development for local and provincial teams.
- Co-developed a Health Network maturity assessment and planning tool.
- Established a common location for key resources on Health Networks (SharePoint site).
- Contributed to creation of Network Profiles.
- Helped establish Health Network geographies.
- Supported mapping and realignment of teams, labour relations strategy, team development, digital health, communications and community engagement.
- Helped align local network development through visual management and pull plans.
