How to Refer
Referrals to the Breast Health Centre are made when your community provider completes the BHC Referral Form and faxes it to our clinic.
What Happens After Your Referral?
Step 1: We Receive Your Referral
Your healthcare provider sends your referral to the Breast Health Centre. Our team reviews the information to ensure we have everything needed to move forward.
Step 2: We Communicate with Your Healthcare Provider
Once your referral has been reviewed, a letter is sent to your referring healthcare provider confirming that your referral has been received. If additional information is needed, we will request it from your provider before proceeding.
Step 3: We Plan Your Next Steps
After your referral is complete and has been reviewed, we determine the most appropriate next step in your care. Depending on your situation, this may include additional imaging, a consultation, or both.
Step 4: We Contact You
Our team will contact you with information about your appointment, test, or consultation. We will provide instructions and let you know what to expect.
Step 5: Meet with Your Care Team
If a consultation is needed, you may meet with one or more members of our team, including a surgeon, nurse navigator, or other healthcare professionals. Together, we will review your results, discuss recommendations, and answer your questions.
Step 6: Personalized Care and Support
If treatment, additional testing or follow-up is required, your care team will work with you to develop a plan that meets your needs. We will help coordinate appointments and connect you with available support services.
