- Miscellaneous Specimen Requisition Form #101878 or SHR Laboratory Medicine Community Laboratory Requisition or Acute Care Phlebotomy Requisition

Specimen Information
Specimen types accepted
- Whole blood
Specimen collection container
- Preferred collection container: Dark Green (Sodium Heparin) 4.0mL
Collection procedure
- Collect on ice
- Send whole blood to RUH Chemistry Lab to prepare washed RBCs
Required volume
- Optimal volume: 4 mL whole blood
- Minimum/pediatric volume: 1 mL whole blood
(Submitting the minimum volume makes it impossible to repeat the test or perform confirmatory/reflex testing. In some situations, a minimum volume may require a second collection.)
Rejection criteria
- Blood collected after blood transfusion
- Specimens received where the proper collection protocols were not followed
- Pathology and Laboratory Medicine Acceptance and Rejection Criteria
